Insurances and Rates:

Dietitian Sandy is a participating provider for the following insurance companies. However, each contract varies based upon the agreement with your employer. Please contact your insurance company to see if you have nutritional counseling as a benefit.

  • Aetna & Meritain Health
  • Cigna
  • Medicare – Referral Required, Diabetes and CKD ONLY
  • Sutter East Bay – Referral Required
  • Hill Physicians Medical Group – Referral Required
  • United Healthcare

For Out of Network Insurances – payment is due at the time of service and a superbill is provided for your convenience.

Office Policies

Here is a brief overview of the practice policies to assist you with common questions and answers.

If you were referred by a Doctor. You may request an appointment above. 

NOTE. A physician referral is NOT a guarantee of coverage by your insurance provider.

Payment Options: Cash, check, credit cards, Venmo are accepted. 

Appointment Cancellation Policy: A 24-hour cancellation policy is upheld. No-shows, same-day cancellations & cancellations made with less than 24 hours notice are subject to A $50 LATE CANCEL OR FULL SESSION fee charged for the visit missed.

We reserve the right to deny further services if a patient does not show to scheduled appointments.

Late Arrival Policy:

Late arrivals will not receive an extension of scheduled service times & you will be financially responsible for full-service fees. If you are 15 min late or more – we will need to reschedule your visit.

Refund Policy:

We do not offer refunds for services or packages, gift cards, or consultations.

Paperwork: Paperwork MUST be completed through your online client portal. You will receive an email to setup your portal. Please have the paperwork completed and submitted online before your initial session unless an alternative has been discussed.
